THERE WILL BE TIME FOR Q&A AFTER<br /><br />Program Information:<br /><br />Target Audience: <br />This activity is designed to meet the educational needs of case managers, nurses, pharmacists, pharmacy technicians, physicians,<br />psychologists, social workers, dietitians, and other health care professionals who are interested in learning about the impact of social<br />determinants of health in cancer care.<br /><br />Learning Objectives: <br />Upon completion of the program, participants should be able to:<br />• Outline the role of SDOH in healthcare and treatment outcomes<br />• Explain the role of SDOH in the prevention of various types of cancer in different populations<br />• Identify and address barriers to the treatment of cancer associated with patient experience and SDOH<br />• Identify and address barriers to integrating social care into cancer care<br />• Describe the current payment landscape for addressing patient social needs (e.g. Z-codes)<br /><br />Joint Accreditation Statement: <br />In support of improving patient care, this activity has been planned and implemented by Partners for Advancing Clinical Education (PACE) and ViralEd, Inc.
